Level I Screening
The Level I screening is submitted online, in the state's PASRR system, by the referring facility. The Level I is a tool that helps identify possible Serious Mental Illness (SMI), Intellectual Disability (ID), or a Related Condition (RC). If the Level I screening is "positive" for one or more of these conditions, a Level II evaluation is required.
As part of the Level II evaluation process, Acentra Health, formerly Kepro, will contact the submitting facility by phone to complete an Initial Screen to determine next steps. If the submitting facility does not participate in the Initial Screening process within 48 hours of completion of the positive Level I screen, the case will be closed.

Level II Evaluation
Level I screens are completed by delegated staff at hospitals and nursing homes. If the Level I screen identifies possible Serious Mental Illness (SMI), Intellectual Disability (ID), or a Related Condition (RC), a Level II evaluation is required prior to admission into a nursing home. The Department of Developmental Services (DDS) is responsible for ID/RC Level II Evaluations. The Department of Health Care Services (DHCS) is responsible for SMI Level II Evaluations and has contracted with Acentra Health, formerly Kepro, to perform these services for the state of California. PASRR Level II evaluations must be completed by Kepro within 72 hours of completion of the positive Level I screen.
As part of the Level II evaluation process, Acentra reviews the positive Level I screen and contacts the submitting facility by phone to complete an Initial Screen to determine if a Level II evaluation is needed. If the Initial Screen confirms the presence or likelihood of serious mental illness, Acentra will schedule and complete the Level II Evaluation. This person-centered, comprehensive assessment is used to determine if the person meets criteria for a serious mental illness, if nursing facility level of services are appropriate, and if specialized services or specialized rehabilitative services are needed while residing in the nursing home. PASRR Level II evaluations are completed through face-to-face or telehealth interview.
Telehealth Requirements
Acentra Health utilizes Microsoft Teams to complete telehealth assessments. Prior to performing the assessment, Acentra will inform the individual of the option to utilize telehealth and obtain written consent to use telehealth. Relevant and current medical records, including written consent, must be submitted by the referring facility within 48 hours of completion of the positive Level I screen or the case will be closed.

Determination
The SMI Determination will be available online and will include placement and treatment recommendations for the individual. The ID/DD/RC Determinations will be issued by DDS, please contact DDS for questions regarding the Determination.
